Wanted: User Library program 0514C (Mastermind) documentation

I'm looking for the documentation for this program. I know that the submission form and program listing are on the museum thumb drive.

My Mastermind program was accepted into the user's library as #514C. Foolishly, I didn't keep a copy of the documentation, so I only have the listing and the submission form. If anyone has this program's documentation, would you be willing to send me a copy?
