Probably the HP-IL Extended ROM by SKWID INK (Jeremy Smith ?) burned into a CMT PROM/EPROM module.

Skwid was (is?) not Jeremy Smith, but rather Ken Emery. An easy mistake though as they were friends back in the early 80's PPC Club meeting days in Orange County.

This module was sold at EduCALC, the manual is available here.

I have been wondering for a long time who was/is Skwid, thanks!
Edit: I just watched the video of his Extended HP-IL module presentation he did at the 1986 CHIP meeting. Cool
