Swap Disk History (looking for)

I am curious about the origin of the swap disks, can one of you provide some history for me? Questions I have: Who made them? How were they distributed? When were they made? "Why?" is obvious, but I'd like to know more about "Why?". How did it get started? Any funny stories (e.g. I drove 100 miles to get a copy of a swap disk, just to find out that it was bad when I got home...).
